Mass. House health care leader has personal experience with medical over-testing

Representative Steven Walsh has attended 800 meetings with health care industry executives while researching the House’s health care cost-control legislation. But his personal experience as the father of a very sick infant has been eye-opening too.

During a breakfast Tuesday sponsored by the Massachusetts Health Council, a non-profit group that promotes public health, Walsh said he witnessed overuse and denial of medical care for his 1-year-old twin son, who was born with a serious heart condition. He was hospitalized more than 90 days during his first year.

“I watched him get over-tested on a pretty regular basis,’’ he told the audience of several hundred insurers, providers, and consumer advocates.

Walsh said he fought back, especially in the emergency room, where it’s hard to get admitted to the hospital without a battery of X-rays and other tests. “On the 50th X-ray, I said enough is enough,’’ he said.

Walsh’s son received most of his care at Boston Children’s Hospital, which he has praised, but he said coordination of care across the health care system could be better.

He said he also witnessed denial of medical care. For example, at one point during his son’s first year, a medical supply company called to say his son had used up his “life-time supply of oxygen.’’ Walsh said his insurance company intervened.

Walsh did not want to go into more detail about his son’s experience to protect his family’s privacy. But he said he is doing much better now.

Over-testing is a key issue in the debate over soaring medical costs. In April, nine medical specialty groups released a list of 45 tests and procedures that patients often do not need – even though doctors routinely perform them. They include annual electrocardiograms, CT scans for low back pain, and chest X-rays before surgery. Excessive use of X-rays in children in particular can raise their risk of cancer later in life.

The House and Senate have passed separate cost-control bills, and both aim to encourage coordination of medical care, in part through widespread adoption of electronic medical records and the development of Accountable Care Organizations — large groups of connected providers that oversee all of a patient’s care.

Walsh and Senator Richard Moore, who oversaw development of the Senate plan, outlined their key strategies to the group Tuesday morning. Moore said the two chambers’ leaders will begin negotiations soon and will present a joint plan to the governor by early or mid-July.

“We need a coordinated health care delivery system that takes out the waste,’’ agreed Dr. JudyAnn Bigby, , secretary of Health and Human Services, who also spoke to the group.

The Cleveland Park restaurant Ripple opened its new artisanal grilled cheese bar Tuesday evening. We were there Monday for a media preview and wanted to share our take on the gooey goodness. after all, the most expensive concoction employs a rare Swiss cheese—challerhocker—with hand-sliced prosciutto and truffle butter to please even the most discriminating turophile.

The decision to open up a grilled cheese bar (just adjacent to the regular bar) was a natural one for owner Roger Marmet.

“I think we have one of the best cheese programs in the city,” he said. And there are no individually wrapped American cheese slices. no Wonder Bread either for that matter. Three cork boards at the front of the restaurant have fancy names thumb-tacked to them. Herbillette. Nuvola di pecora. Winnimere. Those and a dozen others sit under glass on display by the colorfully tiled bar. a Panini press had been collecting dust in the back as the restaurant went in the direction of a chef driven, daily evolving menu featuring seasonal and local ingredients over hot sandwiches. Put those cheeses and that press together, lend a bit of training to turn the cheese monger into a melted cheese monger, and the grilled cheese bar was born.

The grilled cheese menu includes seven sandwiches ranging in price from $8 to $12. the Swiss Bank Account, featuring the aforementioned challerhocker, stands out on account of the generous slathering of truffle butter. a Krusty Krab features lump crab with béchamel and creamy Mt. Tam. the Stinky Pete, with its Spanish torta la serena—a sheep’s milk cheese with notes of thistle and cardoon, asparagus and anchovy—is complex and distinctive. the housemade mozzarella on the Lotsa Mozza doesn’t melt like the others, but still blends well with roasted tomatoes, basil and speck.

Perhaps more fun is checking the boxes on the red and white paper menus to create one’s own concoction. the menu implores the patron: “Don’t make it gross, please stop at two cheeses.” the available cheeses are on the flipside of the menu, categorized by milk type, rind and mold. So I could look to the stinky blues, notice the apple notes in the stichelton and pair it with a strawberry-black pepper jam. “If you dream it,” the menu reads, “we can build it.”

The grilled cheese bar is available every Sunday from 5 to 6:30 p.m. and again from 10:30 to midnight. they don’t come cheap, but we’re not dealing with Kraft singles here! And my sithcelton creation (I also had some washed-rind pichin and pickled onions for good measure) came with six or seven generous slices. to sweeten the deal, the nearly 50 wines by the glass and draft beers are half price during the first seating.

Break from school doesn’t mean break from eating healthy

Summertime — and the living is easy. But getting your kids to stay on a nutritious diet is a different issue completely.

“During the school year, kids are seeing those fruits and vegetables,” said Kristi Murphy, a dietitian and supervisor of the Child Nutrition Department with the Abilene Independent School District. “It’s important for kids to continue eating fruits and vegetables during the summer.”

Late hours and easier access to junk food can derail parents’ efforts to keep their child on healthy eating habits, but Murphy said involving children in the selection of fruits and vegetables could help them eat healthier during the summer.

“One thing you can do is take your child with you to the grocery store and let them help select the fruits and vegetables,” she said. “You can get them to try new things.”

Murphy said people might be surprised at what children may be willing to try and might eat after they eat it a few times.

“When we first started serving sugar snap peas, some of the kids asked, ‘What are these?’ ” she said. “Now they take them. the same with kiwi fruit and whole pears. a lot of the kids weren’t used to seeing whole fruit. a lot of people have the perception that kids don’t want to try anything new, but a lot of times they want to try new foods.”

Murphy said a good way to serve fruits and vegetables is in smoothies. Fruit can cover the taste of some of the vegetables without getting rid of any of the nutrients, she said.

“You can add spinach to a smoothie and the kids will think it’s kind of cool,” Murphy said. “It makes it a Green Monster.”

It was just as important to avoid excessive amounts of some foods, such as fried food and fast food, which may be more accessible in the summer than during the school year, she added.

Murphy said it is important to keep juice, water and low-fat milk on hand for kids rather than sugary drinks. However, she is not among the critics of flavored milks.

“I know there are some people who don’t like the flavored milks because of the sugar, but I think the nutritious benefits of them outweigh the sugar,” she said.

AISD is sponsoring a summer program that provides breakfasts and lunches at 15 locations throughout the city from June 4 to Aug. 10. the meals are available to any child 18 and younger at no cost. However, even if a family doesn’t take advantage of the service, the menus can provide parents with some good ideas about feeding their kids during the summer.

For instance, an easy summer breakfast is peanut butter and jelly between graham crackers and served with milk or juice. String cheese and milk can be a good snack as well as a healthy alternative to chips.

In addition to keeping the kids on a healthy diet, Murphy also recommended physical activity.

“It doesn’t have to be structured exercise,” she said. “Just playing is good. Get them away from the video games and out of the house for a while. Eating and exercise go hand in hand.”

Make sure you watch the portions you consume. Filling up your plate is a habit that many people have, and it’s one you should break.

When it comes to fattening foods, only take a little. you can also put healthier items such as greens on the plate first, leaving less room for high calorie foods. this way you can have some of all of the foods you want to eat without having to worry that you are completely undoing all of the hard work that you have done. Limiting your portions is something to keep in mind at all times, but many people forget about it when November and December roll around. So, rather than having a list of foods you have to stay away from, focus instead on controlling your portion sizes.

You want to be aware of exactly what you’re eating, and writing it down is the best way to do this. when you work out, it’s good to know just how many calories you need to burn off. It’s very helpful when you’re trying to lose weight to know for certain what you have and haven’t eaten. this will make everything much clearer for you, especially those instances when you stray from your diet (as we all do sometimes). what may have just been one cookie will start to feel like a few later on in the day and the guilt from exaggerating your misstep can send you straight to more snacking. yet when you write everything down, you’ll know that a tiny indulgence is just that, and nothing more.
